Confidentiality and Record Keeping
Services are confidential, regardless of who is paying or who referred you.
I may have a duty of care to break confidentiality if I am concerned about your safety, the safety of a child or the safety of others. If there is such an issue, I will aim to discuss this with you. Prior to starting any work together, we will discuss and agree on the confidentiality agreement so that I am sure you understand my terms and conditions.
To aid our processes and maintain professional standards, I will discuss input with my clinical supervisor. You will not be identified as all personal details are anonymous.
I also have a legal obligation to keep computerised and written records of any services and assessments. This information is securely kept. I will use annonymised data for my audit and evaluation purposes.
